This example shows how a Fyneworks enquiry forms can be integrated with any website

The example below renders the form directly on the page

You can also invoke fyneworks programatically by running fyneworks.show()

It can also be loaded as a dialog

PS.: it will also render new forms on demand if the DOM is updated (uses MutationObserver)
